The Evergreen Family Health & Urgent Care Clinic delivers vital medical services to the rural community of Sutherlin, Oregon—an area that has long faced barriers to consistent, accessible care. Conceived through a collaboration between Evergreen Family Medicine, local school districts, and regional health organizations, the clinic addresses both immediate and long-term healthcare needs for Sutherlin and surrounding North Douglas County.
Located on a 5.5-acre site with expansive mountain views, the 16,000 square-foot facility is designed to serve as both a clinical resource and a community hub. It houses family medicine and urgent care services, with embedded radiology, phlebotomy, and lab spaces that enable same-site diagnostics. In addition to providing care within its walls, Evergreen’s outreach extends into local schools and senior facilities, with the building’s dedicated conference and community space supporting educational programming, mobile clinics, and telehealth appointments.
2fORM Architecture’s design approach focused on creating a sense of familiarity, warmth, and dignity for patients. Natural materials, including locally sourced wood, appear throughout the interior and exterior. Post-and-beam detailing references regional construction vernacular, while large windows frame the surrounding landscape—bringing daylight and natural views deep into the space. The resulting environment is one of calm and approachability, intentionally designed to reduce anxiety and encourage engagement.
Flexibility was a driving principle in the layout. The building is designed with a future expansion wing, and programmatic zones are organized to allow the clinic to scale its services in response to growing community needs. Key adjacencies between urgent care, lab, and imaging reduce wait times and improve operational efficiency.
Sustainability and regional investment were deeply embedded in the process. The project prioritized local contractors and materials to support the regional economy and minimize embodied carbon. Energy efficiency informed every systems decision, including a high-performance envelope, daylighting strategies to reduce artificial lighting loads, and an efficient mechanical system tailored to clinic needs.
The Evergreen Clinic is an investment in the long-term health and resilience of a rural community.
